By applying for this course, I confirm that:

I am available to attend the duration of this course.

I have approval from my manager to attend this course.

I understand that places are not given on a first come first served basis, therefore I will not attend the course unless I receive an email confirming my place.

4.

If I have to cancel my place, I will give Playfield Institute as much notice as possible to allow someone else to attend.

5.

If I do not inform Playfield Institute that I cannot attend, I may jeopardise my place on future training.
